  • An illness that requires the patient to be admitted to a hospice and receive specialized care
  • Acute pain that’s expected to last for 2 weeks or longer for an acute condition. This includes a surgical procedure for which a medical professional may generally prescribe opioids for a limited edition.
  • Persistent pain lasting longer than two weeks that’s not adequately managed by conventional medications. The qualified medical provider must ascertain that the condition can only be managed using opioids, opiates, or physical interventions.

Here are the steps to follow:

Step 2: Access the Utah Department of Health’s Medical Cannabis Program Online

After you meet with your qualified medical personnel (QMP), you’re now ready to log into the EVS. 

The EVS, short for Electronic Verification System, refers to the Center for Medical Cannabis database.

It’s the system used to manage and issue medical marijuana cards in Utah.

Now that you already have an account (you opened one when registering for a medical card the first time), you only need to log into the system. 

Click here to access the system, input your username and password, and click Sign in.

Step 3: Initiate the Renewal Process

Once logged in, hover over the Tracking Inbox. You will find it in the top left-hand corner of your screen.

Click on Patient (Not New Patient since you’re already registered).

Make any necessary changes to your patient’s info, then click the Save & Resubmit for Certification button. 

Once you’ve done this successfully, the status will change to “Awaiting Certification.”

At this point, you’ll need to contact and update us so we can initiate the recertification of your medical marijuana card. 

Step 4: Update the Green Team Doctors

At this point, you’ll need to contact and update us so we can initiate the recertification of your medical marijuana card. 

Please note that you can’t proceed to the next step before we’ve recertified your Utah medical card renewal.

Step 5: Pay the EVS Fee

Once we’ve certified your renewal, the EVS will notify you to pay a $15 renewal fee.

To pay, log into your EVS account, go to Tracking Inbox, click your Name, hover over the Payment tab, and then click New Payment.

Once you’ve paid, your status will change to “Awaiting State Approval.”

Step 6: The State Approves Your Card

Once your Utah medical card renewal has been approved, the state will email you a copy of your Utah cannabis card.

You can also access your Utah MMJ card and print it through the EVS portal.

Go back to the Tracking Inbox, click on your Name, and then click the Print Card button.

People between 18 and 20 can qualify by petitioning the Compassionate Use Board.

The board, comprising seven providers, reviews all card applications for people under 21. 

People with conditions not listed on the statute may also petition the board for approval.
